Unlocking for using another carrier SIM is up to your current carrier - Verizon is the only US company I know that is selling unlocked iPhones. ATT requires you to call or fill out an online form to request an unlock. Not sure about others. Normally you can dial *611 for support.
Even an unlocked iPhone still has to be able to use CDMA. So, the iPhone has to have been on a compatible network:
Verizon, Sprint, US Cellular, Alltel, and other small companies.
Verizon will still charge you for a dataplan, which you would want some data to use the phone's features. Depending on the myriad of questions surrounding "unlocked", you will find it easier to go with PagePlusCellular or other small carriers that run run on Verizon's network.
Most of those no-ciontract companies only provide 3G speed on Verizon's network.
Verizon tells you your monthly fee is one thing, then charge you more for supposed federal fees. Most of the smaller carriers actually charge you what you agreed to pay (those federal fees are already included in the stated monthly cost). If the smaller companies can afford to do that, someone's lying about the fees you are being charged!
Depending on how your iPhone was "unlocked", Verizon may be hesitant to put it on their network.
If you get the iPhone working on Verizon, you will probably be happy with the call quality; but unhappy with how poorly it works on you wireless network, unless you have an Apple router.
You would have to contact your carrier to see if they offer a unlock service. After the carrier has confirmed that your phone has been unlocked, you can replace the SIM cards with the card of the other carrier.

As far as I know, carrier locked iPhones need to be jail-broken in order to use another provider. As for your other question, considering that your carrier was verizon and assuming you also purchased your phone from a verizon store, most likely it is CDMA. You can tell by looking on the right side and seeing if there is a SIM card tray, if there is not, it is CDMA. The iPhone 4S hardware is compatible with GSM and CDMA, it depends on the loaded software.
Most carriers offer this service. I know that verizon lets you set the numbers yourself through my verizon, but a quick log on to your online account should present the option to set blocked numbers.
Simply insert your SIM card into your iPhone (with respective carrier IE AT&T in an AT&T iPhone or activate a Verizon iPhone 4/4S on Verizon with a clean ESN). You may need to contact your respective carrier to validate a data plan in order for the iPhone to function properly with data and visual voicemail.
