Hi, This is what I have found at Blackberry's website, let me know if it helped.

How to Enter Unlock SIM card codes on BlackBerry 8100 Pearl:

1. Go to Options > Advanced Options
2. In the SIM Card menu, hold down the shift (to the right of the zero) and type meppd
3. When you see the unlocking level screen, hold the ALT key and type mepp2
   
   Note: You will not see any text while you are typing either of these commands.
   
4. 4) Enter the unlock code.

Go to manual and look up how to set auto lock when in holster, if you disable this feature it should disable the start up code.
