Go to a salvage yard and unbolt the whole hinge assembly.
Just did this on my 2500HD, so I assume its similar. I had to remove the door first. Remove the inside left kick panel and unplug any wiring harnesses coming from the door to the inside of the truck. Pop the rubber flexible boot coming from the door through the access hole behind the kick panel using a screwdriver. Remove the bolts on the hinges and the bolt from the stay arm. The door can now be lifted off. The existing hinge pins have to be ground off on the underside of the pins before they can be punched out from underneath. Putting an oversized socket over the top pin and then havilng a helper hold a sledgehammer on the socket helped when trying to drive the pins up. Once removed, replace the bushings and install the new pins. Using new pins that are threaded and use a locknut will greatly simplify installation. Do NOT use pins that use a clip to hold them in.
