Just get it checked in the customer care, there might be some problem with the device
Evidently the 7940 isn't compatible with the APC-41. The 7942 is, and have a request into IT to see if we have any of those models. Here is the steps to get it working on a 7942 in case someone else needs it:
Software Requirements: The Cisco server must be running Cisco CallManager System Version 4.1(3)SR5x or higher. Verify by logging onto the Callmanager with a browser. Press the detail button on the main page to display the SW version. The Cisco phone must be running firmware 8.3(3)x or higher. Press the settings button on the phone (Checkbox icon). Select "Model information". The firmware revision is listed under "Load File" Phone verification The phone must be set for "Wireless Headset Hookswitch Control" = Enabled Press the settings button on the phone (Checkbox Icon). Select "Device Configuration" >> Select "Media Configuration" If "Wireless Headset Hookswitch Control" is " Disabled", follow the instructions below. Enabling Wireless Headset Hookswitch Control Access the server administrator with a browser. Select Device>Phone through the drop down menus. Click "Find" with no selection criteria to obtain a list of all phones Open the desired phone. Scroll down to "Wireless Headset Hookswitch Control" Set to "Enabled" Click "Apply". Wait for the dialog box to appear, then reset the phone by clicking the reset button. Expansion Modules If the phone is equipped with a model 7914/7915/7916 expansion module: The Expansion module will be plugged into the telephone AUX connector Connect the APC-4 (APC-41) control connection into the expansion module Use a 85638-01 adapter to extend the APC-4 (APC-41) audio input to reach the headset jack on the phone

Make sure the headpiece is charged. Turn the headpiece off by pressing the little black button on top of the earpiece and holding for about 5 seconds. Turn the earpiece back on again the same way. You should have a slow flashing blue light on the mic boom. The base station will now try to find the earpiece and within about 20 seconds the red light on the base should be steady and blue light on top should be on.
If this doesn't work you may need to manually pair the earpiece to the base by pressing and holding the Listen Volume Adjust + Button and headset Call Control Button on the earpiece until the light on the mic boom flashes red and blue repeatedly. As above, the base station will now try to find the earpiece and within about 20 seconds the red light on the base should be steady and blue light on top should be on.

Please note that your hand receiver has to be off the hook when using the headset.
This is correct, additionally you can use a handset lifter accessory, such as the HL1 or HL10.
Subscribing Headset to Base:
The headset and the base unit are supplied subscribed to each other. However, if you wish to use a replacement headset with the base unit (or if you lose your connection to the base unit), then you must re-subscribe as follows:
1. Return the headset to the charging base.
2. Press and hold both the + and - buttons on the back of the charging base until the red light on the front of the base starts to flash.
3. Let go of both buttons. The red light should continue to flash.
4. Press the mute switch (black wheel on the top of the headset) straight down until a green light appears on the headset, then let go of the wheel, and the light should remain on the headset.
5. Resubscription is finished when the red light on the base is fully illuminated and the green light on the headset disappears.
6. Remove the headset from the charging base lift telephone handset/press headset button if not using a handset lifter. The headset and base should now be connected.
7. If this procedure does not resolve your issue to try a system reset.

The only thing is to check the voltage output by the charger... usually 5 volts. The lights MAY blink even if voltlage is too low.
If it is OK, then battery in the headset has failed. Have the battery replaced or DIY if not under warranty.
The usual cause of this is to continue using batteries under load when one of the cells is totally depleted.

This works if you're using the HL10 handset lifter with your CS70. The
HL10 has a built-in microphone to pick up the vibrations from the
ringing of the phone speaker under the handset. The HL10 came with an
accessory package that has an extra round microphone on a cord to help
the lifter if the telephone speaker is located in an out-of-reach
location on the phone. You take that external microphone, plug it into a
jack on the back of the HL10 and let it hang free making sure it's not
resting on any surface. That should eliminate any ringing in your
headset.

Make sure that the Plantronics 500A base inteli-sensor switch is set to
the number one setting (should be set on the one line, not the two line
setting, on the right side of the 500A base unit). Also if you are
using an EHS (Electronic Hook Switch) instead of an HL10 Handset Lifter,
that could be the issue as well. The only other thing is to check the
user guide for the hearing aid and make sure that there is not an "auto
answer" setting that may need to be turned off. Here is a link below for the
