MENU > SETTINGS AND TOOLS > PHONE SETTINGS > SECURITY > (Enter Your Password) (Default password is the last 4 digits of your phone number) > RESTORE DEFAULTS
WARNING: This will completely restore your phone back to factory defaults, if you intend to continue using your phone on your service, you will have to reactivate the phone using the *228 method. I do not know if this will incur charges or require any assitance from Verizon.

Here's the step my friend.
Go to menu - Settings - Phone settings - Security - Reset phone.
Shortcut method: just Press "Menu 9534"
