Does it have a fan? Is it working? If not check it has power and replace it if necessary.
Is the heatsink getting hot? Check there is airflow over the heatsink. If it has enclosed ducting blow any accumulated dust out with compressed air.
If the the heatsink is not getting hot than the thermal bond with the GPU and/or memory is broken. This is the techy bit: if you don't feel comfortable with this either get someone else to do it or buy a new video card.
Go to an electronics shop and buy a tube of heatsink paste. Take the video card out of the computer and remove the heatsink assembly (they are usually secured with four screws and/or plastic clips. Remove the old thermal paste off with a plastic or wood scraper (this is important! you don't want to scratch the chip casing or the copper) and clean it off with alcohol. Drop a little new paste on each surface you cleaned. A little goes a long way here, a lot just makes a big mess when you squeeze the two surfaces together, so go easy with that stuff. Replace the heatsink assembly making sure its good and tight and you should be good to go again.
