A few software based spots you should check out might be your best bet. The double- and sometimes triple- locations for your volume control. If you have a media player running, it might not fully control system sound properties. Check thru control panel and then sound devices to ensure all sound locations are good.
But your real problem I suspect is hardware based. (have to rule out the easy steps first!) My laptop had the same issue, which turned out to be the actual sound card. if your sound changes when you move the output jack or when headphones are plugged in, does it change sound level or speaker output at all?
Corrosion, a cracked sound board, or bad solder connection could all point at being a bad card.
