Definitely your Micro SD Card has a problem. might be ita a virus problem.
Try to format it by your Computer not by your Phone. If you have a AntiVirus scan your Card and then try to format it.
if you are not successful in one chance Don't afraid at all, try it three or four times by removing and then inserting the Card again in the Computer........

there's a card slot on top of the device - see pic attached (which shows both - SIM card and memory card slot opened). To open it press down and release dummy card, thenn pull it out and put SD card into slot (pushing it slightly deeper)
You can back up your Treo to either your PC or your SD
card. If you back up to your PC via hotsync, then there is a nice freeware which
can be installed to your Treo that is called SyncAll (all one has to do is right
before the sync, open the program and click "force all to sync"), and it will move
all the files, including preferences and such that don't normally get sync'd. If
you backup to your SD card, there are numerous programs for accomplishing this. It
is important that the number of backup-to-SD programs are limited for the Treo 650
due the NVFS file system. You can try THIS ONE for example - it's free.

common bug. Turn it off... take out the chip... wait.... turn it on... wait till you see the windows mobile screen not the samsung screen... then put it in... and when it boots up you should see everything.
make sure your card is formatted too. (they usually are)
