First of all go to http://www.sss.gov.ph/ and signup for Member ( it is important that you select member not employee). First register yourself, it will require your SSS number.

Confirm that you're 16 or older and hit "Continue." Complete the personal details for your new account, including your name, gender, date of birth, marital status, occupation, address, phone numbers, email address, preferred language and your tax file number.

Indicate if your account will be owned by just you or jointly with someone else.
Check off if you'd like a confirmation email after you save or submit your application, whether you already have another account with Commonwealth Bank and whether you want card access.

Click the "Marketing Information" box if you want to receive information about new products and services. Hit "Continue."


Complete your application by filling out the details about your deposit account on the next page. Review your online application to confirm that the information you entered is correct and legal and hit "Submit."
Wait for email confirmation and wait for approval of your new account.

Using a credit or debit card is the simplest way to create an iTunes Store account. However, not everyone has one of these cards. Go to paypal site and click problem with log in. In the question what's the issue? click don't know either one.
On the spaces provide 3 email addresses that you have previously used with paypal enter the code and click continue. if any of the email address that you provided matched with the one in paypal's file the email address will receive a link to reset your password. Just follow all the instructions.

If there's no balance or transaction with the forgotten account, you need not to worry, it'll automatically be deactivated after few months but your original/existing account will be functioning properly without any hindrance or obstruction. When an account is not active or transaction being done for several months, paypal will send you an email to your email address with the message describing about the deactivation of the account, if you don't take any action, it'll be deactivated.

It's better if you can at least remember the email address [Note: your username is your email address] of your account so that you can deactivate it yourself. But still you can try to close it using the "Forgotten your email address or password?" form.
