You need to clean the heaphones entry. cut the head of a ear cleaner so it's the size of the hole, with a little cotton left. Dump it in little alcohol and use it clean inside the hole. worked for me like magic.

The problem is with your head phone it may not correctly connected,kindly check it out or replace with the other headphone and check it.

it's probablyjust a defective earspeaker. Bring it to the apple store to see if you geta replacement through them. If you can't, there are third party iPhone repair services, that will fixed it for cheap. Also, I found an DYI iphone repair guide, so you can do it yourself too, not that difficult. Part is available on ebay.
