hi, this is a common problem and usualy happens when some one has entered the wrong security code more than 5 times all you need to do to get this code is call your service provider and then they will give you this code over the phone. They dont charge for this but please dont try guessing the number as its a 15 digit code and getting it wrong more than 5 times means your phone is hard locked and will no longer be able to get unlocked.
So call your service provider tell them your phone is asking for the puk code and then they will give yo this over the phone for free write it down in case you need it in the future.
But remember the code they give you will only work with the sim card you are using it wont work in any other phone it is a sim card code and not the phone code so it will only work with the sim card you are using ok.

PUK CODE: This is usually a problem associated with a LOCKED SIM CARD. This can sometimes happen when you enter your PIN incorrectly too many times. I would recommend going to a retail location of your service provider (ex. AT&T Store) and ask for a SIM CARD SWAP. This service is free for most providers. If you are charged for a SIM swap, tell the person helping you that SIM swaps do not cost any money and that they are free and that is what you were told over the phone.

There are websites online that provide unlocking services for a fee. If you google "phone unlock service" you will see many options.
There is also another way to go. You can call yout service provider (ex. ATT, T-Mobile) or the company that branded the phone (the company that the phone is locked to) and tell them that you would like the unlock code for your phone. They will probably ask WHY you want the unlock code. Tell them that you will be going out of the country and need the phone unlocked in order to use a pre-paid SIM card in the country of destination. They will then get you the unlock code typically within three days. Hope this helps.
No Sir, the PUK code is only under the authority of your service providers.
The only way is to contact the Service providers.
They will first authenticate some info regarding the owner, which should be you, then they will tell you the PUK.
