With your sim came a pin code (you could change that) and a PUK. After you entered a wrong pin in 3 times, the phone is locked. You than use the PUK to reset the PIN.
The puk cam on the came card or paper you initial pin came. (often the card you broke out the SIM)
When you can't find the PUK, ask your provider. And if he can't give a PIK, ask for a new sim they should put your phone on the new sim. Then you have a new pin and a new PUK. Keep the PUK on a safe place, so you don't need the help from your provider next time the pin is entered wrong.
