Puk code and new pin no
PUK stands for Personal Unblocking Key. This is a unique
eight-digit code specific to your SIM card. This is required when the PIN
number to unlock your mobile phone is entered incorrectly 3 times. You are then
given 10 attempts to enter your PUK code. If an incorrect PUK code is entered
after the 10th attempt, you will block your SIM card permanently and will
require a new SIM card from your mobile phone provider.

Your cell phone service provider can give you the PUK code once you give them your account details and they have confirmed your identity.

Only your service provider of your sim card can unlock your puk, it's free once you validate you're the owner

the PUK code is set from entering the wrong sim card pin number to many times.
Call your provider or go into a provider store and they will enter the puk code and set the default sim pin code.
The PUK code can only come from your provider and they don't mind giving it to you.
If you have AT&T you can get it from the website.

No the Default PUK and PIN codes are not set by you, these codes are Service Provider Dependent (SIM)
All you have to so is to call your Service Providers Customer Support (Like of T-Mobile, AT&T...).
And ask them about the PUK and PIN code, they will first Authenticate few things about the Ownership of the SIM card, and will give you the codes, in return.

they will tell you your puk code...........
