PUK code is provided by your cellular network operator, should be on
the same paperwork as your PIN code. It's related to the SIM card, not to the phone. It is used to unlock a SIM card in case the PIN has been entered wrong 3 times. It then allows you to set a new PIN code.
Some operators issue only the PUK number and no predefined PIN, so you can set your own PIN from the start.
If it's not there, or you lost the papers, you'll need to contact the operator's hotline to get it.

SOURCE: My Samsung T528G GSM freezes
The Samsung T528G cellphone, like most cellphones, may freeze when you're using it. When it freezes, the screen doesn't respond when you touch it. You need to perform a soft reset on the phone when the screen freezes to return the touch-screen function. A soft reset is similar to rebooting a computer; none of your data or settings are deleted.
1.Flip the Samsung T528G over so the back is facing up. The battery cover is located on the back of the phone.
2.Press the battery cover release button on the top of the phone and lift the battery cover off of the back of the phone.
3.Remove the battery from the Samsung T528G.
4.Reinstall the battery so the contacts on the battery face the top of the battery compartment, where the phone's battery terminals are located.
5.Slide the bottom of the battery cover over the lower part of the battery compartment and press down on the top of the battery cover until it clicks in place.
6.Press and hold the "Power/Menu Exit" button, which is the button below the screen with the phone handset over a circle with a line in it, until the screen turns on.
If the suggested steps didn't work, I would suggest to contact your manufacturer so that they will replace it for you.

You can get the unlock code through online..........give your IMEI number and get the unlock code for ur mobile.... i will suggest the website Unlock-Zone.com which is doing good service .....you can approach this website.
This site is nearly perfect and they were recommended highly in the cell phone forums. I used them to unlock my cell phone. contacted them and within 30 mins i had my unlock code and my phone was unlocked.
