SOURCE: headphone jack output

The same problem here. I solved it like this:
You must open the device. There will be a PCB with outputs and inputs. You must must detach it by removing screws on outside and then disassemble the PCB from two plastic pins to get the PCB rotated (I used plice to detache it one by one) and the conections of jack socket visible. In my case one od the lanes to the hot pin was broken. I checked that with Ohm meter 'cause it wasn't clearly visible with an eye. So having the PCB more or less secured I started to clean the lane a little bit to have a chance to put the solder layer near the pin and breakage. Get some really precise soldering device. It's a precise job. Do not overheat the diode which the lanes goes to. After placing some of the solder I checked the new connection by placing a jack into the socket and Ohm meter once again. All was perfect and working after assembling. Piece of cake.

How can I get all my velocity pads to work?

After a lot of use chances are the sweat and oil from your fingers has gotten under the button contacts, when that happens the contacts cannot complete the circuit and you have to keep pushing and moving them around trying to make a connection. You need to open the unit and clean all the contacts under the buttons on the circuit board, as well as the bottom of each button. Some rubbing alcohol on a Q-tip works well.

None of the buttons on the device will work. Batteries are fine and blinking light inside appears normal. Cannont lock the device from outside or get the code to unlock the door.

Check to be sure the keypad is still connected to the inside assembly. There is only a one year warranty on the electronics and Kwikset will not sell the electronic parts separately. If you want to replace it, there are other brands that are key-compatible with Kwikset. You'll still have the same problem if the electronics breaks though. The main difference is that other brands don't move the bolt with battery power, they just enable it to be moved by hand.
