Don't know, that is not a computer problem, that is a dental administration problem. Ask your dentist.

I want to renew my loss passport? what will I do?

Things You'll Need
  1. Old passport in original with self-attested photocopy of its first four and last four pages, including ECR/ECNR page.
  2. If the old passport was issued when you were a minor or did not have the ECNR stamp - provide attested copy of matriculation or degree certificate
  3. Proof of address if different than the one on the old passport. Acceptable documents include phone, gas, electricity bill, ration card, spouse or parent's passport copy
  4. Recent passport size photographs (Three) in colour showing frontal view of full face are required.
Steps 1 Re-issue vs. Renew
A new passport is re-issued (not renewed) when a current passport (validity of 10 years or more) is 1 year from expiration or it is less than 3 years after it has expired. A passport issued with a validity of 1-5 years under certain emergency conditions can be renewed at no additional cost. This article covers the re-issuance of a Passport that was issued with 10 years or more of validity.
2 The Application Form
The Application Form for re-issuing a Passport is the same as the one required for a new Passport. You can get the Application Form from any Passport Service Center/Office or any Post Office. You can also use the link below.
3 Fees Required
The application fee for reissue of a passport is the same as a new application. Passport for Adult (36 page) - Rs. 1000, Passport for Adult (60 pages) - Rs. 1500 and Passport for a Minor - Rs. 600. It is better to take extra cash with you when submitting the application as there are additional fees for Police Clearance Certificate/ECNR Reqd, etc.
4 Where to Apply
Completed Passport Application Forms with Supporting documents can be submitted at Passport Offices, District Passport Cells/Collector's Office/DSP's Office or at Speed Post Centers. Please be sure to note down the File Reference Number.
5 How to check status of Application
You can check the status of your application online by providing the file reference number at the Consular and Passport Division Website
Tips
  • If you have a busy lifestyle and do not wish to waste your time, use a reputed Travel Agent who can get you a passport quickly for a small fee. The travel agent will also be able to verify that your documentation is accurate and sufficient.
  • If you have any other questions or situations please use the link below for the official website of the Indian Passport Division.
Caution In case a person has resided at more than 1 place in the past 1 year, then a separate Person Particulars form (part of the Passport Application) needs to be filled out for each address that he has resided at, for the past 1 year.

This article is on how to apply for an Indian Passport Online. It must be remembered that you only register your India Passport application form through this process. But you must in any case visit your respective passport office, to submit the passport application form. The advantage of registering a passport application online is you do not need to obtain a token number for submitting the application at the Regional Passport Office. You don't even have wait long in the queue!!

Registration of passport application forms online can be made for the following.

Download in English (from passport.gov.in)

You are however advised to read all information on the website of your regional passport office before registering your passport application online. You can access these websites through the home page of passport.gov.in here and choose "Select Passport Office for Home Page" from the drop-down.

Next, go here and click Continue (at the bottom of the screen) to register your passport application online with the respective offices.

After registration, you will be asked you to Open/Save your application form. After Open/Save, you will have to print the application form. If you are not able to print the application form for any reason, you must note down the passport application number. With the help of this application number and date of birth, you can re-print the application form later. Note that you can also print the application if you have Saved the application.

After printing the passport application form, fill up the form and visit your Indian passport office to submit it. You will also have to pay the fee, and submit supporting documents such as date of birth certificate etc., along with the passport application form, on the appointed day and time. The date and time of appointment at your jurisdictional Passport office will be informed by the on-line system and will also be printed on the application form!

You should be at your Regional Passport Office, 15 minutes before the appointed time and Proceed to your respective counter. You can them submit your passport application form, along with all supporting documents and pay the fees.

What all I need to get pastport of my son and how can I get an apointment?

First, you need to be somewhere in the country where you have citizenship.
Second, you need to prove your identity, e.g., birth-certificate, photo-ID (driver's license?),
and maybe proof of employment.
Third, you need to find "sponsors" -- people with credentials (doctors, priests, dentists)
who have known you for a few years, and will vouch that you are you.
Fourth, you need to get "passport photos" -- any camera-store will be able to produce these, for a fee.
Fifth, you need to find the nearest Passport Office in your region of your country,
or find the web-site of that Passport Office, and get (or download) a Passport Application Form.
Complete the form, and include the necessary fee for processing.
Then, either mail-in the form, or submit it in person.

Getting a passport for a child is mostly the same, except for what the child cannot provide,
e.g., driver's license, and record-of-employment. :-)

How to get a passport?

  • 1 Fill out the Application for Passport, Form DS-11. You can download this form online and bring it with you when you apply for your passport.
  • 2 Get two passport photos taken. The identical photos must be 2 inches by 2 inches. The pictures must offer a full, frontal view of your face, from the top of your head through your shoulders. The photo must be taken with a plain white background. Many post offices, photo studios and mail box locations take passport photos for a nominal fee.
  • 3 Bring one of the following to the Passport Acceptance Facility to prove citizenship: previous U.S. passport, certified birth certificate, naturalization certificate or a certificate of citizenship.
  • 4 Bring proof of identity. A current driver's license, government or military ID are the most common forms of identification. You can also bring a previous U.S. passport or naturalization certificate.
  • 5 Locate the Passport Acceptance Facility nearest you. Use the U.S. Department of State Web site to find the nearest facility. You simply input your zip code and the Web site will generate a list of acceptance facilities in your area.
  • 6 You must pay both a passport application fee and an execution fee to get your passport. You will turn in these fees when you file your application. It takes about six weeks after filing to receive your U.S. passport. If you are traveling soon, you can pay an additional fee for Expedited Service and get your passport within two weeks.
  • Kind regards
    Andrea
