I cant access open my phone. it requires my PUK code which I have no idea what that is
Call up your network provider for the puk code! PUK code is required when you fail to enter the correct security key asked after the phone is turned on. There is no need to worry on this, simple call up your network provider and they will give you the PUK code for your device.

to enable access to your phone again, please contact your cell phone carrier, they will provide you with a PUK code for your phone to unblock it. They will ask you information about your account(Name, phone type, password,etc.)

contact your service provider and have your imei number from inside your phone at hand because they will ask you for it, and they will provide you with a puk code.

if you have got the package that your sim card came in then u'll find the PUK on there or else just contact the operator and they'll give it to u mostly free of cost
