I take it what you're saying is that you have to feed paper via flipping out the front tray and then pressing "online" or "continue", and that it will not feed paper from the actual paper tray?

You can try to take the beast apart and see why the control panel is not functioning, but I'd sit it on the curb, as it was probably manufactured sometime between 1993 and 1997. time to let the old girl die.

Why has it stopped printing manual feed?

Does the printer try to feed the page and does not?, or does the mechanism not engage at all. If it tries to feed and is unsuccessful then based on the age of the printer likley the manual tray pickup roller is worn out and the separation pad is likley bald as well. You can use the paper path test to confirm if the tray is working at all. That would really be the first place to start.

When printing back side of paper, paper turns sideways and tears in half.

see that u r feeding the oaper straight. Try to feed the paper manually instead from automatic feed. Also remove the curl of the paper form the top end by hand before feeding

Top of form fault

Top of form fault - I assume you hit "Feed" button and still got the form fault... if so:
1. check paper sensor position, sometimes just 1/8" one way or the other will make a difference.
2. calibrate the printer to the stock you have loaded - calibration is EASY, just press and hold the feed button until the printer cycles 8-10 times. Let go, should say "Calibration Complete". Hit "Feed" again to see if your top of form fault is fixed.

Paper jamming

1. Turn the printer off and open the access cover.
2. Look for any obstructions in the paper path and remove them if necessary. (It is possible to move the carrier assembly with the power off.)
3. Turn the printer on, load some paper, and press the Form Feed button.
4. Watch the paper as it feeds through and try to determine where the jam is occurring.
5. Check to see if all the rollers are turning.

If all the rollers are turning, there may be something in the feed path that you can't see. Go to step 6.
If not, there is probably a feed motor or a gear problem and the printer may require service.
6. If the paper is jamming under the carrier assembly, you may be able to fix it using the manual feed slot.
7. Turn the printer off.
8. Take a #10 envelope and feed it through the left side of the manual feed slot until it comes right through the printer to the back of the exit rollers.
9. Move it up and down first, and then slowly move it over to the right side. If something is stuck under the black guide plate, this may work it loose.
