Epsons are sometimes bad about this due to a known paper feed issue, if that is the case there is no other fix than to replace the printer, however if the paper is in as squarely as it can be then fastened in with the slider thats all you got...

Did you try to switch of and on the printer? Could be something is obstructing the print head if the lights come on again. Switch of the printer and check is anything (paper or something else like paperclip or...) is blocking the printer head. Remove the obstruction and switch the printer back on. Epson has great support online that is more than just a manual or a new driver. Visit Epson just Google epson and the product name. skip the selling sites and go straight to Epson.

Are you using an epson genuine inks? actually the things that you have done should resolve the problem, at least 3 cleaning cycles. does it happen on any kind of paper or photo paper that you are using?
Sounds a lot like you have an ink pressure issue and at a guess you are using a CIS (Continuous Ink Supply) system.
I would refer back to the documentation that came with your printer and make sure that you are not overpressuring the printhead with the ink reservoirs set too high or some part of the priming process has been missed.
