The washer fluid comes from the same reservoir as the front, it is routed via tubing to the high mount center brake light lens where there's a spritzer nozzle.
Since the Caravan is front wheel drive, there would be no transmission lines running to the rear of the vehicle. This reduces the possibilities to either a fuel leak, or a brake line. If you smell the fluid you should be able to tell immediately if it were gasoline. This would be a dangerous thing. Fuel leaks must be isolated and repaired quickly to avoid risk of fire. If it were brake fluid, and the leak were stead, you should, if you inspect the brake master cylinder reservoir, a lower level of fluid than normal, possibly even a total loss of fluid in the rear chamber. If so, then the brake line to the rear has probably ruptured from age/corrosion. It must be repaired or replaced.

I wouldn't think so, the windshield and rear window washer are operated with independent pumps in their respective reservoir. I would check fluid levels and washer fuse(s)
camillachimi... yes, take off the front passenger wheel and you then can get to the inspection plate that covers the washer pumps. i am having the exact same problem as you on my 2001 caravan sport... rear washer works but the front washer pump doesn't appear to be getting any power... fuses looked good... even replaced the pump... still not working. have you found out anything more?
check the transmission fluid level - be sure - VERY SURE - not to over fill it as draining the oil is easy - draining the trans is way more expensive and you will kill it if left over full
