It would be best if you posted up the p codes. Usually the misfires are cause by bad coils. I would start by changing spark plugs. then seeing if that fixes any of the misses. Then I would "borrow" or buy new coils(the things that go over the spark plugs) and see if that solves the problem. If you replace coils and still happening, then look at the wire harness as it gets old and brittle and cracks.
With the car running rough it could cause the harsh shift but if it is staying at 5000 rpms then the ****** is about to go out on you. The rough idle sounds like a vacuum leak. If you can keep the car running at idle listen for a sucking type sound. That would indicate where the vacuum leak is.
Most likely cause is a crack in the distributor cap. This allows moisture to penetrate the cap and causing misfires and stalling. This would be the 1st place to check.
