If your PUK code is blocked, you need to replace your SIM card. PUK is needed when you block your PIN (3 failed attempts). However another 10 failed attempts will lock your SIM permanently which, according to your description, seems to be the case.

The reason why your phone is bloked is the 3consecutive incorrect password that you entered in your phone.PUK code will be coming from your mobile service provider. They have the PUK codes stored in their database. Its 8 digit number that you should enter on the PUK field.

PUK stands for Personal Unblocking Key. This is a uniqueeight-digit code specific to your SIM card. This is required when the PINnumber to unlock your mobile phone is entered incorrectly 3 times. To get your PUK Code you should Call costumer care (Service Provider) And Ask Them For PUK Code They will give you the PUK Code.
You can not call From your Mobile so use the your friend or any relatives number which belongs to your service provider's network & call them & ask PUK Code they will give you.
or visit your network provider center for example if your using Vodafone Sim Then Visit Vodafone Stores (Vodafone Care) they will provide you the PUK Code.

Hi, In this please do contact your service provider and inform them that the PIN is blocked and the phone is asking for a PUK code for your SIM card and they would provide you with the information once you help them with the SIM card number located on the SIM card itself.

The PUK & PIN codes are Security Codes that are on the SIM card. PIN( Personal Identification Number ) PUK ( Pin Unlock Key). when the PIN code in entred wrong the SIM automatically Blocks the PIN. to unlock the PIN you need to Enter the PUK. but if you enter the PUK wrong the Entire SIM is Locked.

The PIN Code & the PUK codes are Only Known by your Network Provider. you have to get in touch with them to get the codes. how ever if you have PUK blocked you will have to get a replacement SIM

. If you try pin more than few predefined no of times your phone will ask PUK code which can also be given only by your service provider which is specific for each and every mobile number . Donot try PUK code many times or else your sim will be blocked and you will need a new sim.
