For this you will unfortunately almost definately have to ring either your provider or Nokia to sort this out. This has happened to me before. Unless you can find the ORIGINAL little booklet thing you should have got when you purchased the phone (if it was new) that had your SIM card and phone number on it. I believe that should have the PUK code on it. If not, just give them a ring. Most operate 24/7. Hope this helps :)

PUK stands for Personal Unblocking Key. This is a unique eight digit code specific to your SIM card. This is required when the PIN number to unlock your mobile phone is entered incorrectly 3 times. You are then given 10 attempts to enter your PUK code.
