I am so sorry your phone is requesting a PUK code. I hope you did not put in other codes after it asked for the PUK code. If you didn't, you are safe! Then, contact your service provider. They'll provide you with the PUK code. Your nokia will then ask you to type in a new PIN code. twice! Make it easy for yourself! If, you did put in the PUK code incorrectly too many times, you just may have a paper weight in your hands. You can contact www.nokiausa.com/support get their 1-800 number to have your phone FLASHED. PUK codes are not like security codes that are phone dependent. PUK codes are rather SIM CARD or mobile number dependent. There are two ways to obtain this, one is by calling your service provider. Two, if you still have the sim pack, this the pack that included the sim card and some relevant info from your provider, you can get the PUK code from there. It is on the card where you took out the sim.

i cant tell you the puk code by telling network provider. You can call to your network provider customer care and give your sim number that is written in your sim card. They will give you puk code. By using that you can unlock your sim card....Hope you will succeed,...
Typically when a phone is asking for a PUK code it means you have tried to edit or set the secuirty pin code. Unfortunately these codes are already predefined by the sim card and the service provider. It gives you the sensation that you can set or change the code but you cannot. You get 3 attempts to enter the correct code after which the phone asks for a PUK code. When it asks for the PUK you also get three tries to undo it. Again PUK codes are predefined by the sim card and the service provider. If you call your service providers technical support number or customer service number they can provide you with the PUK or the PIN code.

You can only get the PUK digits from your network service provider. But you know there are some service centers that are retrieving PUK digits- but I'm pretty sure they have contacts to the network. Try to go there but I'm sure they will charge you for that kind of service. Trust me I did it once too.
