I have a HL-R6167W. The sound works. Menu works and TV Guide is fine, but the picture is a plaid green and blue lines. I have tried several input sources.

the fan like or high pitched sound may be a color wheel--if you unplug the set and remove the lamp you can look at it--any pieces of the colored plastic wheel missing or broken?
Check if it has a scrambled image on all sources ie - DVD- VCR - cable/satellite box. Since the PIP is normal it sounds like the signal source is bad.

You need to do an aggressive reset on the TV. This solved problem
completely for me. You must unplug the TV when it is on. Repeat this
several times until the TV is jolted back into the factory default
settings. The issue is actually with the programming I believe. The
clue was that the display worked 100% perfect in HDMI mode, and the
problem only existed in the cable setting, no matter if wires were
attached or not.
Again, do an aggressive reset to the TV by unplugging it several times
while it is on. It may not register the reset at first because after
all, the actual problem is a programming issue and it's having trouble
recognizing commands in the first place.
Hi Dave I am Vortash if you replaced the Colour wheel and the problem went but then has returned I think it may just be a bad connection on the board check the ribbons from the lcds are secure and the plug slide that holds them in is all the way closed , also check the rest of the circuit for dry joints , I dont think this is as bad as you fear .. Some Samsungs have a problem if the set is turned on bfore the input put device due to a firmware issue. Try turning on the device (game, DVD, etc) BEFORE turning on the TV. Let me know if this helped.
You are absolutely correct, When anyone complains of vertical lines or bars, the next step is to see if it happens on only digital inputs ( digital board ) , only analog inputs ( analog board ) , or both ( DMD board ) .
