It may be many things, here some thing to try:
the ink or toner is contrammed with foreign liqued like oils, water, bad ink, bad toner cartridge,
oil, water, ink toner inside the ink itself
oil from a human skin my have came in contact with refill of ink or toner cartridge, cleaning solvent to clean the cartridge, needles for hte ink or toner cartridge, water in the ink or toner cartridge, steam or vapors of some kind, compress air can or air compressor will spread water solution, the freon is used in compressed air can thats way they get so cold when you spray the can.
paper may also be wet some how try new and different brand of paper.

Go to Control Panel->Printers. Here right click on your printer and select Troubleshoot. There you will be asked about alignment. Say yes to it and follow the instructions.
Also you can Open Help from Printers and type the requirement Aligning Print head. In the adjoining window the necessary portion will open.

if the test page is printing black and colors fine then the problem is with the software. since u r saying that all the softwares are giving same results, i think that the black cartridge may be empty. is this cartridge is a refilled one or else it needs cleaning. try running cleaning cycles two or three times. but don't run more than that otherwise u will lose ink. if possible clean the nozzle part with a very very soft cloth very lightly with no force applying. do not do this every time other wise the head will be gone.
try to reset your printer-- 1) With the printer powered off but connected to a power source, press the Power button while pressing and holding the Stop/ Reset button. 2) When the Power is lit, press the Stop/Reset button two times while pressing and holding the Power button. 3) When releasing the Power button and the Stop / Reset button (regardless of order), the printer will move into a service mode. (Waiting for menu selection) 4) When the on/off lights in green, press the Stop/Reset button the specified number of time(s) according to the function listed in the table below.(Each time the Stop/Reset button is pressed, the LED lights alternately in orange and green, starting with orange.)note: press 4 times for waste ink counter reset.

use a flash light check all paper path, specially the area around the toner. all you need is a small piece or other (dust). There is a micro switch that detect the paper travel. so if anything preventing the switch to open, you will get error message.
after that, open the toner door, reseat the toner and close the door. the pirnter should return to normal.
I figured it out! For all those who have problems clicking it into place it's probably the printer's not on. Turn it on an the top half of the cartridge holder which is connected to the printing head should slide to the left part of the printer when it's on, then slide the cartridges in an click in.
