You have 2 identical numbers save from your phone book. check carefully each numbers you have just save, coz your phone cant show which name to appear every time you have a call. If the problem still persist check the phone contact, and check the settings. try to check if your using both phone memory and sim card memory at once. Just select only one if phone memory or sim card memory to use as phone book storage. thanks

Please call the Customer Care of your Network Operator (sim card provider), they will tell your the code. There's no other way or no means to get the code except from the Service Operator. They might ask your few questions related to the Sim Card, like your name or your address, tell them promptly and they will give you the puk code for your sim card. It's also important to note down the Sim Card Number (number written on the sim card) if in case they ask you the sim number. You will also be assisted on how to disable the security settings.
Note: if you keep entering random/unknown numbers/codes, the sim card will be locked and will be rejected. It'll not be usable anymore. Good luck.
Ring your service provider if you got it through them and tell them you lost your cell phone and give them the IMEI number. They will barr it from being used and the IMEI will be blacklisted amongst other telecom companies. Check your IMEI number on the box of your cell phone.
If you didn't record your IMEI number prior to losing your phone, the best you can do is ring your service provider tell them to cancel your SIM card to prevent it from being used. You can ask them for a replacement SIM card later on with your exact number if you do eventually find your phone or get a new one.
Just a reminder, its always good to record your IMEI number through pressing *#06# on your keypad. Helps in the event that your phone is lost/stolen.
