If you had smoke thats not a good sign
after you cleaned fan and heatsink do you properly
replace anything you may have removed.
The card is more then likely blown i would replace and not try to use it

From your post, it seems like Windows is booting correctly, but your card isn't correctly displaying the signal once the machine tries to load into your user profile.
Go into Safe Mode and uninstall the drivers for the card. Go back to the beginning of the installation process and follow it step-by-step. If this does not help, try inserting the card first, and then installing the drivers afterwards.
