For the Basic FSSAI Registration, the documentations required are given below:
a. Government issued Photo ID.
b. Photo of FBO
c. Scanned copy of the signature.
You can also apply for the fresh registration online from the online portal of FSSAI.
I encountered the same problem, just last night. If it is the same, the only way I could get it to turn off was by taking the batteries out and the lens still would not retract. After re-installing the batteries several times with the same result, it would not turn off, I did this:
I re-installed the batteries and powered up the camera, I applied a little force/pressure to the lens and it went back in. After that the camera seems to be working fine.
