Welcome to Fixya.
To make the phone ask for the PUK code, the sim lock has to be enabled.
To lock the SIM card, perform the following steps:
1. From any Home screen, tap the Menu key.
2. Tap Settings.
3. Tap Location and security.
4. Scroll to and tap Set up SIM card lock.
5. Tap Lock SIM card to select and enable the SIM lock.
6. Enter the SIM PIN.
7. Tap OK.
The default pin is 1234.
Now, whenever you will turn off then on your phone, it will ask for a SIM unlock code. Just incorrectly enter the code and it will be PUK locked.

My friend you need to call your service provider to get the PUK of your number.

it is possible that your sim card provider has locked your account because of several missed attempt to unlock the sim on a new phone. all you need to do is call your telco provider and ask help to reset your PUK code. Goodluck!

hopefully you are Sprint client........ have them do transfer all your contacts for you, it will be done 30 - 60 mins depending on how many you have and how many customers are ahead of you
