If you have 220 volts at the poles on the upper heating element and it doesn't heat up, it has to be the element or a loose connection where the power hooks up to it. If after replacing the element and the power was applied to the element before the tank filled back up with water, the element probably burned out again. The tank must be full before turning power on to the tank.

If wire is bad between upper and lower parts of water heater, and water heater has tight-fitting 2" foam insulation, it usually indicates water heater replacement.
