The headphone icon means that you are connected to either a headset or bluetooth for external audio.
If these are not plugged in or running, it would be the headset connector is stuck, thinking that you have a headset still plugged in.
Unfortunately you wil need to visit a service centre for this.
Try turning off all wireless, incl bluetooth and see if this goes away
