You need to contact your cellular service provider to get the PUK code.
I would advise that you do not attempt to enter any more codes until you have spoken to your service provider, because phones can be permanently disabled by repeatedly entering the incorrect code.

Unfortunately, your going to have to call the Manufacturer or your phone to solve this issue. The Manufacturer of your phone would be Nokia, LG, Samsung, Motorola. They are the only ones who have access to this information. Some places may charge you to get the unlock code but most won’t so it’s really hit and miss. Nokia charges 14 dollars. In this case you need to contact. If your looking at your phone screen and it’s asking you for a security code, you may want to try the last 4 digits of your cell phone number, that is the default for most phones. If that does not work then you can try 9999 or 0000. Those are also standard with most phone companies. If after that you still have no luck, call your service provider to get help with the code otherwise you risk locking your phone out permanently which stops you from being able to receive phone calls, text messages, etc. I’m sorry your having so much trouble with this! PUK means "personal unlocking key" .
The PUK code is asked if you enter a wrong pin 3 times.
You need to call the phone company that released the SIM card, they can retrieve the PUK code from a database for free.
In your case you need to contact your SIM card provider technical support to unlock the SIM.
The PIN and PUK are related to the SIM card, not to the phone.
If you replace the SIM card with a different one, then the phone will not ask for the PUK anymore.

puk code has to be given by sevice provider , you have to call to your service provider ( sim company ) and tell them the sim number then they will give you PUK code
