Format the card using a Memory card reader, and format it to FAT file system.

Take the SIM-card out. It's important that the SIM card IS NOT in the phone, during the following steps!!
Now you have to enter the whole code on your phone

1.gif Enter "#"
2.gif Enter "*" three times to write "p"
3.gif Enter "*" four times to write "w"
4.gif Enter "*" twice to write "+"
5.gif Enter the 15 numbers 227105652573201
6.gif Enter "*" twice to write "+"
7.gif Enter "1"
8.gif Enter "#"

Your Nokia phone is now unlocked.
