Why are you trying to load it in DOS? Turn your computer on, and let it open to XP sp2. Once you done that, load the SP if that's what your using, and it should ask you if you want to upgrade it. Or, just go to HERE. and have it downloaded and upgraded for you.

the video card you used have a capability for dual monitor..so 1st thing that you need to do is to make sure that the computer recognized the 2nd monitor and activate dual monitor capability on the nvidia panel..after that on your tv select the proper source(like VGA, HDMI1 etc.) on which you'll be able to see the the video from the computer to your TV..

Use an adapter power cable, and have two 4-pin standard Peripheral power cables.
NOW, I hope your Power Supply will handle the additional load.
