I assume you mean your SD card. In that case just go to any department store (or CVS, Walgreens etc...) and pick up a new one, there cheap these days (depending on the size) you should be able to pick one up for about $15 - $20 for 8GB. Put the new card in your phone, every phone is different but it is usually on the side of the phone or in the battery compartment. The phone will format it if needed and you can start loading new contacts, pictures, etc...
If you talking about a Sim card, then you might need to go to the phone retailer (Verizon, ATT, Virgin Mobile, etc...)
