puk code is provided with your sim card. when you buy a line they give a small card where you find your pin number, also there is a puk number on it which is necessary if you lock your pin number.

This phone is only Tri-band and made for Europe only, it is not Quad band which means it has the 2 European frequencies but only the one odd US one so you probably cannot get access because the phone is missing the frequency.
