It is most likely the way the bowl and lid are set.
Facing the machine: the bowl locks in, with the handle to your right directly above the switches. The lid has a double tab...see underside of the top of the lid. Place the lid on tightly with both tabs locked into bowls handle. It really clicks in.
I overlooked this too. Set into the foam packing material is a stem made just for the shredding disk. It's about 4" long and also has a hex shaft. This hex is just slightly smaller than the hex on the other shaft the drives the blades. The dough hook - Remove the blade assembly and put the dough hook in its place.

Place the gearbox on top of the blender/food processor
Place the work bowl over the spindle, with the handle facing the front of the machine - make sure the tabs lock into place.
Place the stem adapter on top of the spindle, then the blade.
Place the lid on top of the bowl, with the feed tube facing the back of the food processor. Then the pusher into the feed tube.
