Boy, I spent some time on this and - so far as finding an exact replacement on the Internet - I'm stumped.
But I'll tell you what I'd do, in your position.
I'd take the old worn (or split) gasket to Ace Hardware or True Value, and ask someone to match it up for you. Really, a gasket is a gasket. It's just a matter of the size, the thickness, and the "compressability" of the rubber (obviously, you'll want a soft rubber gasket). If you find one, you may want to buy two of them... just in case.
If this doesn't work, then get a square piece of rubber gasket material (Ace, True Value, Home Depot, etc.). You can either cut one yourself (use cups, jars, measuring spoons that are the right diameters to draw your two ring patterns), using an Exacto knife to carefully cut them out.
If you don't want to do this yourself, ask someone who's handy & who has a steady hand to do it for you. You can pay them with a freshly made smoothie.
