Try *#7370# to set it to factory default. All your thied party SW and settings will be removed and your phone will become as if it is brand new. SO remove your card from it before you dial this code. Also set the profile as General.
Camera failed generally means theres a loose flex or damage to one or more part of the camera, if you know how to dissasemble a phone check the flex cable. if not send it back under warranty or insurance.
File Manager will work (Start -> File Manager). I use Total Command as opposed to file manager. There is also a setting to default your (future) pics to the SD card. Hit the camera button, hit Menu (right soft button), select Options... select Camera, then Save Files to.
This happens also because the RAM memory is full by programs opened before. For example, whenever you turn of any program or aplication like contacts or messages, it doesnt turn off at all by clicking the X, it remains opened and you have to go to Star, Set up, then click on Memory, then on running programs, and stop them. This way ram will be able to use the camera and store pics on storage card. Do this also when you see that camera looks like unfocused, its always the ram memory.
