MSA 30 unplugged HDD

I have a HP DL580 G4 connected to a MSA 30 dual channel (Smart Array 6404). On each channel there is configured RAID 1+0 with hot spare. The drives are Ultra320 SCSI 72GB, 15K running firmware version HPB1 or HPB6. If I try to start the server with one HDD unplugged, the ADU is reporting both the unplugged and the hot spare as Missing or Bad HDD and the array cannot be rebuild. Any help would be appreciated.

I am getting logical disk on host twice

Are you sure that our logical disk is not mapped twice to your host channel on your MSA 1500? If it isn't have you tried a power cycle to the storage array and a clean shutdown/restart on your host?
