If you are are confident enough to have ago you can take it apart you will need a stardriver and a bit of patience when you take the cover off there will be connecting strips from the keypad back(3 of them), 2 at the top and 1 at the lower side right, genly pop them off.
This will give access to the rear of the keypad where you can blow dust from the roller ball mechanism, gently wipe the sensor on the motherboard and replace reverse order, grease and debrit gather in the mechanism from pockets etc then the use of the ball pushes it inside, hope this helps.
Have you tried cleaning it? The best way I clean mine is using a q-tip, dabbing it in rubbing alcohol, be sure to squeeze the excess out of the q-tip, and roll the wet q-tip all around the ball. Make sure you roll the ball so that all of it gets the alcohol on it. You can use the other side of the q-tip (dry side) to dry the ball. Hunnybunny11

Try going into your applications screen, Which can also be the home screen if you have your theme set for it. Try clicking on your blackberry button, the one to the left of your trackball, which will bring up a small menu, scroll to show all and click your track ball. Then you need to find the address book icon and click your blackberry button. If hide is checked in the menu uncheck it by clicking your track ball again. This should unhide it and make it available on your screen again.
This can only be resolved by having a service book sent to you. you will need to contact your service provider's Adavanced Tech dept. Let them know you've lost your address book icon and need the service book sent to your phone. they will be able to access the blackberry site and have the service book sent to your device.
