Thanks for the suggestion. Cleaning lens did not work. Next on the list to try is:
1. Dirty or damaged objective lens.
2. Loading (mechanical) not completed reliably.
3. Damaged lens suspension or damaged lens cover preventing free movement.
4. Dirt, gummed up lubrication, or damage in sled drive mechanism.
5. Dirty/defective limit switch or sensor.
6. Defective spindle motor.
7. Spindle table height incorrectly set.
8. Bad component in optical pickup.
9. Cracks in ribbon cable to optical pickup.
10. Need to adjust servo (or less likely, optical) alignment.
11. Faulty power supply, electronics, or control logic.
12. Bad connections including missing/erratic optical deck shield.
13. External interference.

- CD lens cleaner did not work. Removed the top of the player to get to the lens. Used a special cleaning solution (Isopropyl alcohol might work), cleaned the lens. Works fine now.
