The little plastic notch (button looking thing) on the back of the phone, near the top slides up (use your fingernail) When this is done the aluminum plate on the back comes off. Then use your fingernail again to pull outward on the top-left notch of the battery.

On Verizon, sometimes you need to update the carrier settings. This is done over the air. Dial * 2 2 8 and press Send on your phone. It will go through an automatic process to refresh and reactivate so just be patient and listen....
I had the same problem with mine... I found that you must let the battery completely drain out; I was continuously jumping to charge my phone when it was down to one bar, and realized that upon doing this, the battery never fully retains its charge for as long as it will if you let it die out (blank screen; completely off).
I did this twice, and have not had a problem since.
