The symptoms you're describing with your 2KD Hilux engine (weak engine with mass smoke, vibrations on rev, and less power) could be caused by several different issues. Here are a few possibilities:
Excessive engine oil consumption: If the engine is consuming too much oil, it could be burning the oil and creating a lot of smoke. This could be due to worn engine components such as pistons, rings or valve seals.
Clogged air filter: A clogged air filter can restrict the airflow to the engine, causing it to run poorly and produce more smoke.

Sounds like bad engine/trans mounts. The more load you put on it the more vibration engine makes. If mounts are bad, vibration will transfer to body and steering

Poor tuning of the engine can result in higher engine vibration. So the fuel and ignition timing MUST be trimmed to allow the engine to tick with ease. if this wont happen the health of the engine is under question and must be checked.
So check the fuel quality, filter and calibration- right fuel mixture of air/fuel.
Next the ignition timing to coincide with the correct timing mark and set using a strobe.
If this wont correct the valve settings, compression and timing must be rechecked.
I am afraid not. The water from the radiator never goes into the engine. Radiator is just there to keep the engine temperature Normal. That is to say that it keeps in cool because engines get very hot while driving the car. If you take out the water, it would overhead quickly and would eventually halt the engine.
As for the vibration from engine, it because the supports of the engine aren't bolted together snuggly. So when you accelerate, the engine vibrates hard and then also shake up the supports and cause noise.
If you don't have to be moving to get the vibration then it has to be either something is not balanced properly (torque converter may be bad, front pulley, engine flywheel cracked) or you have a bad mount, causing the vibration from making the engine or trans contact the car body in a place you can't see.
Hard to pinpoint without feeling it myself. I can say for sure it's not the cam belt, unless there's debris stuck to the inside of the cam gear causing the vibration.
