First, I would suspect a bad battery. Try changing it with a known good battery. Check the metal contacts on the phone and the battery to make sure they are clean and damage free.

Charger seems to have both red and green lites flashing at the same time no sequence and both batteries connected read 4.0 VDC, are these chargers repariable or throw aways

Your manual is here. The section on charging states, that green blinking means your battery is fully charged but blinking red means that your charger is faulty. I would say repairable.

1. Plug the female connector of the AC power cord (supplied) to the AC receptacle on the charger and plug in the male connector on the AC power cord into the wall outlet.

2. The On/Off LED indicator is blinking RED, this is abnormal. Unplug AC power cord from the on-board battery charger and wall outlet. Charger may need to be replaced. Contact an Invacare dealer or qualified technician.

3. When the On/Off LED indicator light is Off, charger is Off.

4. When the Charge LED indicator light is YELLOW, the batteries are charging.

5. When the Charge LED indicator light is solid GREEN, the batteries are fully charged (as their condition will allow).

6. When charging is complete, unplug the male connector of the AC power cord from the wall outlet and then unplug the female connector of the AC power cord from the AC receptacle on the charger.

Why does my nextbook 8 when fully charged show an exclamation point in the battery? and when I unplug it it totally drains the battery?

Can mean the battery isn't communicating its status to the tablet--failing battery, bad connection, etc. Talk to tech support for a replacement if it's still under warranty. If it's not and you can still charge it to 100%, hang on and hope for the best.

iPod Symbol Meanings
The Apple iPod personal media player uses a set of symbols to communicate what's happening to the device: when it needs to be recharged or when it's experiencing problems.
Sometimes it's not obvious what these symbols mean; problems with your iPod may cause conflicting or confusing icons to be displayed.
Folder with an Exclamation Point

This suggests that your iPod can't read data stored on it.
Your iPod may simply not have enough charge in its battery to read its hard drive.
If recharging it doesn't help, you can try connecting it to your computer, then updating it with the latest version of the iPod's software.
If none of this works, you may have a dead hard drive; your iPod will need to be repaired.
Battery with an Exclamation Point
A battery with an exclamation point usually just means that your iPod needs to recharge.
Once the battery has charged up again, everything should be fine.
You can charge an iPod through a FireWire connection to a computer, but not if the computer is in sleep mode or switched off.
Third-generation iPods cannot be charged via USB 2; later iPod models can, however.
If you are having problems charging your iPod, try switching cables.
You can also try resetting the iPod by pressing Play and Select together (Menu and Play on earlier models).
If none of these methods help, your battery may need to be replaced.
Empty Battery
If the battery icon shows an empty battery, this means that your iPod is almost out of charge and needs to power up.
If you know that the iPod has been charging for a while, and the battery icon is still empty, you may need to update its software.
Older software can lead to a confused battery readout.
Wall Plug with Power Adapter
This means that your iPod needs to be plugged into an electricity supply while it updates its firmware.
You won't be able to do anything else with it until you do this.
Frowning IPod Icon with an Exclamation Point
A picture of an iPod with a sad face and x-ed out eyes plus an exclamation point indicates a very serious problem with the device.
In some cases, draining the battery completely then recharging and resetting the iPod can offer a short term solution, buying you enough time to back up your iPod's data.
The underlying problem will remain and is likely to get worse until your iPod is repaired.
Apple Logo
A brief appearance by the Apple logo when the iPod is reset or switched on is normal and nothing to worry about.
If the logo does not go away and the iPod doesn't boot up, there is a problem.
You can often resolve this by putting the ipod in Disk Mode and then restoring or updating its software; note that you will lose all data stored on the iPod if you do this.
If this doesn't work, your iPod needs specialist attention.
Do Not Disconnect
It is not safe to unplug your iPod when you see this icon. Wait till it goes away, then you can safely disconnect your iPod.
Spinning Disk Icons
The spinning disk and magnifying glass icon means that the iPod is performing a disk scan.
If you don't want to perform a scan right now you can re-set the iPod; you'll see a disk with a cross when you stop the scan.
If you would rather let the scan complete, you should connect the iPod to a power source as scanning drains the battery.
If the scan finds issues but can fix them, you'll see a disk plus an arrow.
If everything is alright, you'll see a disk plus a checkmark.
If there is a problem that the scan disk function could not fix, you'll see a disk plus an exclamation point.

What is the proper way to charge the Pronto M51

Eduardo After using your chair and the battery level lights are down with yellow or red lights showing just plug the chair into the wall socket and let er rip. Now really after using your chair during the day and you are ready for bed just plug in the charger and let it charge over night. In the morning unplug the charger and you should be set for the day. If you don't use the chair everyday always put it up with a full charge and about once a week plug it in and let it charge overnight. Never leave the chair plugged in all the time and never leave it for more that 24 hours. The charger has two lights on it one red and one yellow or green that are hard to see because they are under the rear cover. When the chair is fully charged the yellow light turns green and you can unplug the charger and you should have a full charge. Hope this helps you

My glowbouy flashes green and red...what does this mean

According to the owners manual for these swimming pool lights, this is what each colored indicator means. Hope this helps.

Solid green
The battery pack is fully charged.
Slow flashing green
The battery pack is almost fully charged.
Slow flashing yellow
The battery pack is half charged.
Medium flashing yellow
The battery pack is less than half charged.
Fast flashing yellow
The battery pack has almost no charge.
Fast flashing red
A charging error has occurred and the light should be unplugged and then plugged back in. If the Fast Flashing Red persists after several attempts, the battery condition is questionable.

Also in the trouble shooting guide, it mentions this:

Problem: Red Indicator Light Is Flashing While Charging

Solution:
A red flashing indicator light when charging indicates a selftest
error condition. Unplug the product and plug it in again.
Try this several times; if the indicator light continues to flash
red, unplug the GlowBuoy and take it to an authorized service
depot.

Battery won't charge on my ryobi 14.4v cordless drill yellow and green lights stay on

I had the same problem and found a solution on another forum...
Plug the battery into the charger. The yellow and green lights come on. Leave the battery plugged into the charger. Plug and unplug the charger power cord from it's power source a few times until the the red charger light comes on..... problem solved!

I think the "smart" circuit in the charger that is supposed to trickle charge before the regular charge cycle needs at least some battery power to test and start the cycle. If the battery is totally discharged, the test can't deternine the battery status and will not charge.... Plugging and unplugging the charger seems to bypass this teat and charge the battery normally. The charging process should work normally after this method.

Battery light blinking red during charging

Dell or Panasonic? Your post has both listed.

If the blinking light on a Dell is yellow or amber or orange (somewhere in that color range) and continues to blink the entire time it is plugged into AC power (wall outlet) and when you unplug it from the wall the laptop immediately dies .... then you battery is dead and needs replaced.

If the blinking light stops after a while (perhaps a minute?) and then turns green while plugged into AC power, then it is successfully charging - or so it thinks.

When you unplug it from the wall, does it stay up and running?
If it runs, it's OK - just ignore the light and plan on getting a new battery one day, start browsing now. :)
If it dies immediately upon unplugging, then your battery is dead and needs replaced.
