I bought the Advanced or High Definition AV pack, but I still don't get Dolby Digital output.

Boot the Xbox without a game in it to launch the Xbox Dashboard. Once you're in the Dash, select Settings, then Audio. You should see options for both Dolby Digital & DTS. Select whichever of these encoded formats your receiver supports & choose 'yes'.
