Your modem diagnostic screen should be located at http://192.168.100.1/
This is usually the default diagnostic for most cable modems, assuming it can sync.
Also, looking at the housing and labels on the modem should alert you to the model number. Ignore anything like a Serial number, DOCSIS number, or CableMAC number when looking for this.

look if you are sure about that your modem is motorola do one thing unscrew your pc casing, shut down your pc , plug out the modem card and you will find too many chips on it see there you will fine something in number like xt9530 moto 6587 that is the best your you can define your modem model ..
