The card slots from recent nintendo's have been adjusted to avoid the use of r4ds. If your nintendo is newer you can try to put a small pice of carton behind the r4ds card in order to push it well against the contacts.

I have three DS Nintendos and all three have a RD4S. One of them had exactly the same problem after a few months. The adapter that holds the Micro SD card shows bad connectivity after frequent plugging and unplugging. I returned the Adapter (RD4S) and got a new one. That was a newer version and since then (over 8 months now) it still runs great.
