Only thing you can do is call your carrier and they can unlock it.. next time wright it down or use your bday or someone close to you bday date myself.. I just use 1111 ..

If it's the SIM card PIN that you forgot, there is no way to unlock it except if you have the PUK.
Usually when you get an new SIM card, you get with it 2 codes, the PIN and the PUK. So when you forgot and you enter a wrong PIN 3 times. at that time the SIM card will ask you for a PUK code. be careful, because if you enter the a wrong PUK 3 times. there will be noway to unlock it even if you contact the service provider.
Now, if you don't have your PUK code or if your GSM service provide didn't provide you the PUK. you can simply give a call to their customer service and they send you the PUK via SMS.

The PUK code is a code associated with the SIM card and not with the phone.
You
have only two ways to get rid of the PUK code. Either you replace the
SIM card , or you call phone provider that issued the sim cadr (eg.
At&T, Virgin, Tmobile , Vodafone, Telecom etc.)
PUK means "personal unlocking key" .
The PUK code is asked if you enter a wrong pin 3 times.
You need to call the phone company that released the SIM card, they can retrieve the PUK code from a database for free.
In your case you need to contact your SIM card provider technical support to unlock the SIM.
